Health Coaching to support Health Inequalities

Buckinghamshire is widely known as an affluent county with great outcomes. However, we know that this overall picture masks some significant variations, with some areas experiencing substantial hardship in their health and wellbeing.  We want to provide health coaching skills to those people that have contact with our service users to enable them to better support our residents.


We are offering you the chance to complete a 4-day Advantage Level 4, Personalised Care Institute (PCI) Accredited Health Coaching programme. 

What is Health Coaching?


Health coaching is a supported self-management intervention. It focuses on supporting people to make more informed and conscious choices about their health. It enables people to develop the knowledge, skills, and confidence to take opportunities to become active participants in their care and wellbeing.

What are Health Inequalities?


Health inequalities are unfair and avoidable differences in health across the population, and between different groups within society. These include how long people are likely to live, the health conditions they may experience and the care that is available to them.

  • The programme initially focuses on the key wards for health inequalities in Aylesbury, High Wycombe, and Chesham.

  • The cohorts:

    There will be two cohorts run in 2024 to support Health Inequalities within Buckinghamshire. Each cohort will consist of 30 people from across the system including but not limited to:

    • Social workers
    • Social care
    • Health care professionals
    • Voluntary sector / charity 
    • Religious leaders
    • Anyone that has a touch point with a service user in Buckinghamshire
  • This programme offers the opportunity to:

    • Collaborate with others from across Buckinghamshire  
    • Develop your coaching skills 
    • Learn the theory, skills and knowledge to support service users with Health Coaching
    • Learn new ways to have conversations and empower others
  • Modules will include:

    • Comprehensive Model of Personalised Care
    • Health inequalities and health coaching
    • Principles of a coaching mindset to empower service users
    • Health coaching models to support self-management
    • Structuring conversations using a coaching approach
    • Practice in a safe and supportive environment
    • Key communication skills that build trust and rapport
    • Goal setting that is sensitive to needs of the service user
    • Skills and confidence to support behaviour change
    • Motivational interviewing
    • Change management tools
    • Stress management and looking after ourselves
    • Peer support, supervision, and a community of practice 
  • Ongoing support:

    Following the 4 days of face-to-face training there will be continued support that includes:


    Community of Practice monthly on MS teams:  

    • support completion of portfolio of work
    • provide additional tools and techniques
    • sharing of best practice 
    • Support relationship building across the system

    Project manager:

    • support needs outside of training 
  • What's required from you?

    • Complete  pre-course reading (30 minutes)
    • Attend four days face-to-face training
    • Complete self-directed study (1hr)
    • Read the NHS Long Term Plan overview (30 minutes)
    • Complete portfolio of work including 6 health coaching conversations (4hrs)
    • Attend some community of practice sessions
    • A change sponsor from your place of work to support you and the organisation embed the learning
    • Case study presentation at the graduation demonstrating how you have used health coaching skills to support a service user
    • The most important requirement is to encourage you to use your health coaching skills in your everyday conversations

    Cohort 2 (4 days):

    Time: 09:30 - 16:30

    Dates: 1st & 2nd  October 2024, 15th & 16th October 2024

    Location: Missenden Abbey


    Graduation: 

    Time: 13:30 – 15:30

    Date: Thursday 5th December 2024
